Orchestrate Convert API
Standardize uncoded clinical terms and convert patient records across formats like FHIR, CDA, and HL7 with automatic terminology enrichment.
Overview
The Orchestrate Convert API, developed by CareEvolution, is a clinical data transformation tool designed for healthcare IT teams, developers, payers, health systems, health tech vendors, and patient data aggregators. It standardizes, enriches, and converts patient data from diverse source formats into industry-standard formats, enabling interoperability across healthcare applications and systems.
The Convert API maps uncoded, proprietary-coded, or free text clinical terms from patient records to industry code sets including ICD-10-CM, SNOMED, and LOINC. It consolidates duplicate records into clean, organized patient records with summaries covering conditions, diagnoses, procedures, tests, and medications. The API has been developed and refined over 15+ years and is reported to support data for over 200 million lives across payers, health systems, and health tech organizations throughout the US.
Core Capabilities
- Maps uncoded, free text, and proprietary-coded clinical terms to standard code sets such as ICD-10-CM, SNOMED, LOINC, RxNorm, and CVX
- Consolidates and deduplicates multiple patient records into a single FHIR bundle
- Converts files into formats including FHIR (R4), CDA, PDF, HTML, X12, HL7, NEMSIS, and OMOP
- Reduces the burden of medical record reviews by organizing patient data with condition, diagnosis, procedure, test, and medication summaries
- Extracts dates and times from non-standard formats
- Handles errors with high ingestion success rates, issuing warnings rather than failures
- Accommodates wide source variations including Epic, Allscripts, Athena, Cerner, and others
- Handles non-standard and proprietary templates across all CDA types
- Continuously updates to reflect changes across industry sources
Supported Conversion Operations
- HL7 to FHIR (R4): Converts one or more HL7v2 messages into a FHIR R4 bundle
- CDA to FHIR (R4): Converts and enriches a CDA document, deduplicating content into a FHIR R4 bundle
- X12 to FHIR (R4): Converts an X12 document into a FHIR R4 bundle
- Combine FHIR (R4) Bundles: Aggregates information from multiple FHIR R4 bundles for the same patient into a single bundle
- Update FHIR Version: Upgrades bundles from DSTU2 or STU3 to FHIR R4
- FHIR (R4) to CDA: Converts a FHIR R4 bundle into an aggregated CDA document
- FHIR (R4) to OMOP: Converts a FHIR R4 bundle into the OMOP Common Data Model format, suitable for applications that prefer tabular data
- FHIR (R4) to HealthLake: Converts a FHIR R4 bundle into a format compatible with the Amazon HealthLake analysis platform
- CDA to PDF: Converts a CDA document into a PDF document
- CDA to HTML: Converts a CDA document into human-readable HTML
Real-World Use Cases
- A large payvider deployed the Convert API to ingest, aggregate, and enrich patient data from multiple sources, enabling real-time access to lab and imaging reports, medication details, claims, and clinical encounter information, as well as real-time acute and post-acute care tracking with FHIR notifications
- A national health plan used the API to create consolidated and enriched master patient records for 46+ million members, connecting over 2,800 providers, 240 data sources, and 7.5 billion claims, supporting AI, machine learning, risk adjustment, quality programs, care management, and clinical trials
- A health tech vendor ingesting records from over 600,000 providers used the Convert API to reduce file sizes and eliminate redundant content; one patient record originally spanning 7,342 pages was consolidated into a single 600-page deduplicated PDF
- A patient data aggregator used the API to harmonize, normalize, and output patient records in multiple formats including FHIR, CDA, and OMOP across numerous markets and operational departments
Architecture and Security
- Cloud-native, stateless architecture that integrates into existing workflows without changes to data infrastructure; API calls can be implemented within hours
- Self-service APIs designed to integrate into existing technology stacks
- Meets HIPAA compliance standards and is HITRUST certified
- Security program based on NIST SP 800-53 Rev 5 at the FISMA Moderate and Privacy baselines
- Regularly assessed by a Third Party Assessment Organization (3PAO)
The Convert API is part of CareEvolution's broader Orchestrate suite, which also includes a Terminology API for standardizing healthcare codes, an Insight API for HCC risk adjustment profiling, an Identity API for privacy-preserving record linking, and Identity Lite for patient record matching. CareEvolution offers a complimentary clinical data diagnostic report assessing up to 1,000 CDAs.
