This technology allows healthcare organizations to implement and scale digital care programs for chronic conditions, promoting proactive interventions. Currently, the company operates remote monitoring programs in Portugal and France, and is expanding into Switzerland and Germany, with plans for a German launch in late 2026. In Romania, they are also establishing a local presence.

Hope Care claims its programs have led to significant clinical and economic benefits, including a reported 76% reduction in hospital days for chronic obstructive pulmonary disease patients, translating to substantial savings. As European healthcare systems grapple with chronic disease management, Hope Care is positioned as a key player, combining clinical evidence with regulatory compliance to enhance patient outcomes and the sustainability of healthcare delivery across the continent.
